Use of the services means acceptance of the Terms and Conditions. Every lab order generated provides the ability to view current terms and conditions.
These terms and conditions are a legally binding agreement between you and Find Lab Testing a partner of Lab Testing API Inc a Delaware incorporated corporation a subsidiary of Lab Testing API LTD a UK registered company. By using the Find Lab Testing service you are stating that you have viewed the terms and conditions and the privacy policy on the website and that you understand and have agreed to be bound by all the terms and conditions. If you do not agree, you should not use this service.
You are acquiring testing for your own personal health knowledge and use.
The service is provided at YOUR request and this service does NOT create a doctor / patient relationship in any way.
The service will NOT provide specific medical advice that includes diagnosis and treatment about your lab testing results. It is your responsibility to discuss your results with a health provider.
ALWAYS SEEK ADVISEMENT FROM A TRAINED HEALTH PROFESSIONAL FOR MEDICAL ADVICE, DIAGNOSIS AND TREATMENT.
Both the ordering physician and testing laboratories are independent contractors with whom Find Lab Testing, and Lab Testing API, INC makes arrangements for your blood tests. LTA will not be held liable for any acts or omissions of the physician, the testing lab, or their agents or employees.

You understand and agree that: Neither Find Lab Testing, Lab Testing API, or third party providers utilised to provide the lab testing service available at Find Lab Testing, nor their respective directors, officers, employees, agents, contractors, subsidiaries, or affiliated companies, will be liable for any indirect, incidental, punitive, special, consequential, or exemplary damages or loss of any kind from using the Find Lab Testing service, or for any damages in excess of the amount paid for the specific item purchased.
The content and service provided are for informational purposes only. The service is not intended to be a substitute for professional medical advice, diagnosis or treat. Always seek advice from your physician, and share your results with them, or other qualified health providers. The Find Lab Testing system is subject to copyright and shall not be copied. The site and all its components content, features, functionality including but not limited to all information, software, text, displays, images, video, audio and its design are owned by it's partner Lab Testing API, its licensors, or other service providers that create the Lab Testing API service. You must not reproduce, distribute, modify, create works of Lab Testing API content without permission.
Prices are stated in US dollars. Payments can be made by credit, debit card, HSA, FSA cards and Paypal. Find Lab Testing reserves the right to refuse any payment made in these forms. Money will be provided at the point of sale. Payment details are held by third party providers and not Find Lab Testing or Lab Testing API. Your statutory rights are not affected by these terms.
Your credit card company may refuse payment to Find Lab Testing/Lab Testing API. If this happens please reach out to them and assure the credit card company that you wish to proceed with the payment. Refunds are processed according to our refund policy. Please see below.
When an order is placed you will have 7 days to request a refund. If requiring a refund later than 7 days you will have up to 30 days to request a refund less a $15 admin fee for the lab order generation. This service is provided at the point of sale and Find Lab Testing a partner of Lab Testing API have 7 days to request a refund for your lab order from the physicians network providing your lab order. For orders older than 30 days you will be allowed a credit on the account to use at a later date.

No refunds will be given to those who visit a patient service center and provide their specimen for testing. No refunds will be provided if patient has multiple lab orders and the wrong lab order was taken to the lab and testing carried out.
Lab orders are valid for up to 6 months from the purchase date. Lab orders older than 6 months are automatically purged from our system. If you’d like to reinstate an expired lab order, you will be required to pay a $15 fee and the difference in test price, if applicable.
You are free to stop using our service at any time. If you do not comply with the terms we have the right to suspend service.
We reserve the right to change our terms and conditions. We do provide the option each time a lab order is placed to view the terms and we assume you have read them when placing an order. Terms are always available at www.findlabtesting.com/terms We will notify where possible of term changes.
Before any lab testing service can be provided consent must be provided by the person obtaining the lab work. This consent will map out those who help provide the service. If you do not agree to the consent you are unable to use the service. Without consent you will not be allowed to create an account. You have the right to remove consent at any point by contacting Find Lab Testing / Lab Testing API.
Some tests provided by the labs have follow up reflex testing to confirm positive results. By accepting the terms and conditions you accept reflex testing to be automatically carried out. If you do not wish to have reflex testing you are unable to purchase a test that could have a reflex test.
You can only use the service if you are older than 18. You must not place a lab order for another person and attempt to change the date of birth at a later date. If this happens Find Lab Testing / Lab Testing API have the right to suspend your account for misuse based on our terms and conditions.
Find Lab Testing/Lab Testing API is currently unable to provide services in the state of New York, Rhode Island and New Jersey. This is due to current state law.
Results are provided in a PDF format that can be shared with your physician. Please note that Find Lab Testing have given you the functionality to do this but Find Lab Testing is not actively sharing your results for you. If you do not wish to consent to share results do not use this function.
If you have been introduced to Find Lab Testing electronically via your health provider they will give us your name, date of birth, email address and the tests that you may be interested in. You have complete control to add and change any tests you wish. You also have the ability to choose if you share your results with your health provider.
I hereby authorize (i) Find Lab Testing (ii)Lab Testing API Inc (with its affiliates and staff, “Lab Testing API”) and (iii) PWN Remote Care Services, (with its affiliates and administrative services provider, “PWN”), including its physicians, their staff, agents and designees, including PWNHealth, LLC, as agent and administrative services provider and the laboratories that perform services requested by or consented by me (“PWN Parties”) to use and disclose health information about me in the manner and for the purposes stated below.
This authorization applies to the use and disclosure of the following information about me: all information in requests(s) submitted by me or about me with my consent and the laboratory test values/results/information which are the result of the request(s) so submitted.
For avoidance of doubt, I specifically authorize the transfer and release of this information to, between and among myself and the following individuals, organizations and their representatives:

This authorization evidences my informed decision to allow release of the information to the parties referenced in this authorization. This authorization is effective immediately and will expire ten years after the date of this authorization.
Upon my written request, I may inspect or copy the information that I have permitted to be used or disclosed, if permitted by law. PWN Parties or Lab Testing API may receive payment or other remuneration related to the use and disclosures herein.
I understand that I have a right to receive a copy of this authorization. I have the right to refuse to agree to this authorization in which case my refusal may affect the services provided to me. When my information is used or disclosed pursuant to this authorization, it may be subject to re-disclosure by the recipient and may no longer be protected by certain privacy laws. I have the right to revoke this authorization in writing at any time, except that the revocation will not apply to any information already disclosed by the parties referenced in this authorization have acted in reliance upon this authorization. I understand that I have a right to receive a copy of this authorization. I have the right to refuse to agree to this authorization in which case my refusal may affect the services provided to me. When my information is used or disclosed pursuant to this authorization, it may be subject to re-disclosure by the recipient and may no longer be protected by certain privacy laws. I have the right to revoke this authorization in writing at any time, except that the revocation will not apply to any information already disclosed by the parties referenced in this authorization have acted in reliance upon this authorization.
